Circumstances

In the yard of the workhouse whilst demonstrating new equipment he ascended a ladder with another fireman when it broke in two and both men fell to the ground. Smith struck his head on the stone floor. Both men were taken to St Batholomew's Hospital where Smith was found to have died from a fractured skull.
